The MTA's engineers noticed there was a defect in the ceiling but did nothing about it, according to the IG's report.

A coworker apparently left a note after the damaged cord was found, 'To my fellow crazy co-workers: If you don't want cameras in the booths and wherever you work, don't cut the cord again.'

In its sweeping report recommending how the MTA can restore public trust, the open-government advocacy group Reinvent Albany suggested the Inspector General’s office could play a larger oversight role.
